The following materials are usually required to replace the Chinese five-star card (new version of alien permanent residence identity card):
1. The original and photocopy of the current valid permanent residence permit;
2. The original and photocopy of the current valid passport;
3. Two recent 2-inch color photos with white background;
4. Valid foreign passport (if there are languages other than English on the front page of the passport, translation is required);
5. Foreigner’s permanent residence certificate;
6. Personal resume of the applicant, requiring consecutive years and months;
7. Proof materials before and after changes in relevant information (submitted for content changes);
8. A certificate of loss issued by the public security organ or a written and signed statement of loss (submitted for lost documents);
9. Information Collection Form for Applying for Permanent Residence Identity Card in China (to be collected at the acceptance window).
The usual processing location is the immigration reception center in various regions. It should be noted that certificates issued by relevant foreign institutions must be authenticated by the Chinese embassy or consulate in that country. Foreign language materials must be translated into Chinese and stamped with the official seal of the translation company on the translated documents and copies of the translation company’s business license. In addition, for special application cases, the interpretation of the immigration management department shall prevail.
